Stenting, TAVR Make Sense Packaged Together for Some Patients

Med Page Today

(MedPage Today) -- LONDON -- Some individuals already undergoing transcatheter aortic valve replacement (TAVR, also TAVI) might as well get coronary lesions revascularized around the same time, according to the NOTION-3 randomized trial. Clinical.

Valvular and perivalvular thrombus after TAVR: Risks and Outcomes

Cardiology Update

Transcatheter aortic valve replacement (TAVR) is the standard treatment for severe aortic stenosis, but post-TAVR leaflet thrombus, identified by hypoattenuated leaflet thickening (HALT), poses potential risks like cerebral thromboembolic events. Perivalvular thrombus has also been reported. Original article: Choi Y et al.

TMVR after TA-TAVR: a re-redo surgery—case report

Frontiers in Cardiovascular Medicine

Prior transapical transcatheter aortic valve replacement (TA-TAVR) may complicate the procedure and is therefore considered a relative contraindication. He had already undergone cardiac surgery in the form of coronary artery bypass grafting at the age of 64 and TA-TAVR at 79 years.

Modified percutaneous coronary intervention-derived risk models (PARIS and CREDO-Kyoto integer scoring systems) applied to Japanese transcatheter aortic valve replacement patients

Open Heart

Objective Postprocedural ischaemic and bleeding risks after transcatheter aortic valve replacement (TAVR) remain a major concern. We aimed to assess the accuracy of percutaneous coronary intervention (PCI)-derived models and the performance of a recalibrated model that included variables more applicable to TAVR.

Abstract 4145631: A Rare Case of Sequential Impella Mechanical Failures due to Infective Endocarditis Vegetations

Circulation

This was treated with a drug-eluting stent, but TIMI 3 flow was not achieved. Due to ongoing shock despite initial mechanical support, the patient was escalated to an Impella CP device after a transthoracic echo confirmed no left ventricle thrombus. Once stabilized, intravascular ultrasound showed significant thrombus and plaque in the LAD.

Highlights of ACC 2024

Cardiology Update

SMART 4 ( NCT04722250 ) studied patients with severe aortic stenosis and a small aortic annulus who underwent transcatheter aortic valve replacement (TAVR). A total of 716 patients underwent TAVR with either a self-expanding supraannular valve or a balloon-expandable valve.
