The University of Minnesota partnered with the M Health Fairview (MHFV) System to incorporate stroke clinical trials into the annual comprehensive stroke education courses. The number of attendees for each session from 2019-June 2023 were recorded along with anonymous comments about the presentation.
These patients must undergo appropriate medical assessment, receive treatment for risk factors, and be educated on how to reduce their risk for stroke. A significant portion of the evaluation may be safely conducted on an outpatient basis, if urgent follow-up care is readily available.
The Neuroscience Nurse Navigator Program [NNN-P], implemented December 2019, included SP who received Alteplase [IV-A] or Mechanical Thrombectomy [MT]. Individualized education included importance of f/u w/SSS & PCP & risk factor [RF] management, with barriers to care assessed.
Nurse navigators have been utilized in other specialties resulting in positive impacts on patient care. Nurse navigators are becoming an important part of Stroke Center teams. Responsibilities vary, including patient education and post-discharge phone calls. Additional high risk patients can be added at discretion.
Electrocardiography and New York Heart Association class evaluations are mostly performed once a year by GPs (46%) and at every outpatient visit by specialists (40%). Further efforts should be placed in educational activities and structuring of nurse-led clinics.
In this pilot project, we aimed to incorporate the Stroke Advanced Practice Nurses (APNs) in one of the Community Hospital (OCH) so that stroke care gaps can be identified and to optimize stroke care.Methodology:The Stroke APN obtained their clinical privileges to work in CH.
Individuals were screened following outpatient referral to Leeds Teaching Hospitals NHS Trust for symptomatic AF. Patients randomised to clinical follow-up received nurse-led telephone calls every two weeks, while those in the app arm received automated reminder messages every two weeks.
Neuro-telemetry nurses designed Stroke 90 as a follow-up outreach program to reduce the stroke readmission average by 2% over six months and address knowledge gaps. The next six weeks reinforce individualized stroke education and reassess for continued post-discharge needs. Outpatient visits and procedural admissions were excluded.
Ultimately, these devices unlock a level of remote monitoring that could play a more direct role in facilitating successful outpatient care. In some cases, wearable devices may help reduce disparities in care and may help improve wellness.
