A Win

Dr. Malcolm Kendrick

21st October 2024 Finally, the dust settles. Zoë and I won our case against the Mail on Sunday. The judgement came out a few weeks ago, but the legal wrangling continued – and still continues. The Mail on Sunday did print an apology. Legally, they do not have to print an apology, but they did, which says something. There is still a statement in open court to come (SIOC).

Risk stratification by renal function and NYHA class in patients with hypotension initiated on sacubitril/valsartan: a retrospective cohort study from 17 centres in Japan

Open Heart

Background Patients with heart failure exhibiting low systolic blood pressure (SBP) have a poor prognosis. Sacubitril/valsartan reduces cardiovascular events; however, its use in patients with low SBP has not been fully examined. Therefore, in this study, we aimed to investigate the association between baseline SBP and adverse events (AEs) in patients starting sacubitril/valsartan therapy using data from a real-world registry in Japan.

Procedural Characteristics, Safety and Clinical Outcomes of Pulsed-field Ablation for Pulmonary Vein Isolation in Patients with a Left Common Pulmonary Vein

HeartRhythm

Pulmonary vein isolation (PVI) by pulsed-field ablation (PFA) has demonstrated encouraging clinical results for the treatment of atrial fibrillation (AF)1,2. Whereas the ability of other single-shot devices, such as cryoballoon-ablation systems, to accommodate for anatomical variations of the pulmonary veins (PV) is limited,3–5 it is currently unknown if PFA is suited for PV variants like a left common pulmonary vein (LCPV).

Atrial Flutter and Sick Sinus Syndrome

HeartRhythm

Sick sinus syndrome (SSS) is a common condition resulting in reduced quality of life, syncope, and pacemaker (PPM) implantation, but predictors have not been elucidated. While atrial arrhythmias are frequently associated with SSS, we hypothesized that atrial flutter (AFL) would strongly predict SSS given shared relationships with right atrial, and particularly crista terminalis, fibrosis.

Personalized Ablation Strategies Optimize First Pass Isolation and Minimize Pulmonary Vein Reconnection During Paroxysmal Atrial Fibrillation Ablation

HeartRhythm

While atrial fibrillation (AF) ablation has advanced to improve safety, success rates have been slower to improve. Factors contributing to this include the failure to personalize ablation strategies for individuals. It has been shown that a lack of first-pass isolation (FPI) and the occurrence of acute pulmonary vein reconnection (PVR) predict AF recurrence.(1,2) Despite this, no patient-specific factors have been consistently identified to predict either of these outcomes.

Efficacy and limitation of nonparoxysmal atrial fibrillation ablation in patients with heart failure with preserved ejection fraction

Journal of Cardiovascular Electrophysiology

Abstract Introduction Catheter ablation for atrial fibrillation (AF) reduces heart failure (HF) hospitalization in patients with HF with preserved ejection fraction (HFpEF). However, the long-term outcomes and subclinical HF after nonparoxysmal AF ablation in HFpEF patients have not been fully evaluated. Methods and Results One-hundred-ninety nonparoxysmal AF patients with left ventricular ejection fraction ≥50% who underwent first-time AF ablation were studied.

In contrast, risk stratification schemes for adverse cardiac outcomes associated with AF, such as stroke, and predictors for AF recurrence post-ablation have recei
