This is a value typical for a large subacute MI, n ormal value 48 hours after myocardial infarction is associated with Post-Infarction Regional Pericarditis ( PIRP ). PIRP is strongly associated with myocardial rupture. The VSR is what is causing the cardiogenic shock! Heart rate was in the 80s. Hammill SC.

The diagnostic coronary angiogram identified only minimal coronary artery disease, but there was a severely calcified, ‘immobile’ aortic valve. Aortic angiogram did not reveal aortic dissection. If you see this, you should Doppler the valve. Fundamentally, cardiogenic shock is an issue of decreased cardiac output.
