Impact of clinical variables on outcomes in refractory cardiac arrest patients undergoing extracorporeal cardiopulmonary resuscitation

Frontiers in Cardiovascular Medicine

Background In the past two decades, extracorporeal resuscitation (ECPR) has been increasingly used in the management of refractory cardiac arrest (CA) patients. Methods All patients who underwent E-CPR between January 2014 and December 2021 at the Montreal Heart Institute were included in this retrospective analysis.

Abstract 4137905: The association between prehospital epinephrine administration and short-term outcomes in patients with shockable out-of-hospital cardiac arrest and extracorporeal cardiopulmonary resuscitation: a propensity matched analysis

Circulation

Background:In out-of-hospital cardiac arrest (OHCA) patients with an initial shockable rhythm, epinephrine increases the likelihood of return of spontaneous circulation (ROSC), but its effect on neurological outcome remains uncertain. Stroke: Vascular and Interventional Neurology

Compared with 2012‐2014, the 2015‐2019 cohort showed increased odds of ICH and shock while the odds of DVT, pulmonary embolism, pneumonia, and UTI were significantly lower. Odds of a mild disability outcome increased from 16% to 20%, OR 0.65 (0.57, 0.74), while mortality decreased from 15% to 12%, OR 0.69 (0.61, 0.78).

Ken Grauer, MD

Unfortunately, before this could be accomplished — the patient went into cardiac arrest. She was successfully resuscitated — with a post-arrest rhythm similar to that seen in Figure-1. Cardiac cath did not reveal significant coronary disease! I supplement this material with the following 2 PDF handouts.

90 year old with acute chest and epigastric pain, and diffuse ST depression with reciprocal STE in aVR: activate the cath lab?

Dr. Smith's ECG Blog

Methods STEMI activations between January 2014 and April 2018 at the University of Arizona Medical Center were identified. Thirty-six patients (36%) presented with cardiac arrest, and 78% (28/36) underwent emergent angiography. Results Emergent angiography was performed in 80% (79/99) of patients.

ECG Blog #388 — Why Does Lead V1 Look Funny?

Ken Grauer, MD

The ECG in Figure-1 was obtained from an 18-year old woman — who moments before been resuscitated from out-of-hospital cardiac arrest. Does this ECG in Figure-1 provide clue(s) to the etiology of this patient's cardiac arrest? I suspected the answer resides in the reason why an 18-year woman might have a cardiac arrest.
