BackgroundProteinuria is a risk factor for cardiovascular events, but its prognostic value for sudden cardiacarrest (SCA) in young individuals remains unproven.
Uncontrolled coronary spasm may be associated with serious arrhythmias , including cardiacarrest ( Looi et al — Postgrad Med, 2012 ; Tan et al — Eur Heart J Case Rep, 2018 ; Chevalier et al — JACC, 1998 ; Rodriguez-Manero — EP Europace, 2018 ). As a result — an ICD may need to be considered in selected cases.
The ECG in Figure-1 was obtained from an 18-year old woman — who moments before been resuscitated from out-of-hospital cardiacarrest. Does this ECG in Figure-1 provide clue(s) to the etiology of this patient's cardiacarrest? I suspected the answer resides in the reason why an 18-year woman might have a cardiacarrest.
In 2012, she had a ventricular fibrillation cardiacarrest. In 1973 she underwent a Mustard operation, switching atrial blood flow using a baffle, which was complicated by complete heart block. An epicardial pacemaker was implanted. The superior limb of the baffle was stenosed and required stenting.
We analyze the national trends in mechanical thrombectomy use and outcomes for stroke five years after publication of the US guideline update.MethodsWe analyzed the National Inpatient Sample from 2012‐2019. ICD‐9 and ICD‐10 codes identified Ischemic stroke and mechanical thrombectomy patients.
We aim to describe the clinical characteristics, functional status and RT use of PHT Pts with stroke during the PHT admission. Methods:Retrospective cohort study of consecutive PHT Pts at a tertiary center between 09/01/2012 and 09/30/2022. Additionally, 38(45%) were ischemic and 34(40%) were hemorrhagic strokes.Of
Smith: This bizarre ECG looks like a post cardiacarrest ECG with probable acidosis or hyperkalemia in addition to OMI. The patient died of cardiogenic shock within 24 hours despite mechanical circulatory support. Below the J-point is marked out showing the ST pathologic deviations. What was the pH and K? Potassium 4,6.
These include ( among others ) — acute febrile illness — variations in autonomic tone ( as may occur with syncope ) — hypothermia — ischemia or infarction — cardiacarrest — and electrolyte disorders ( especially hyperkalemia — but also hypokalemia/hyponatremia ). Circulation, 117, 1890–1893. [3]: 4]: Antzelevitch C and Brugada R.
This ECG pattern may be diagnostic of B rugada S yndrome IF seen in association with: i ) a history of cardiacarrest; polymorphic VT; or of non-vagal syncope; and / or ii ) a positive family history of sudden death at an early age; and / or iii ) a similar ECG in relatives. Electrocardiol 45:433-442, 2012 ).
In addition to a spontaneous or induced Brugada-1 ECG pattern, criteria for B rugada S yndrome require one or more of the following: History of cardiacarrest, of polymorphoic VT, or of non-vagal syncope — positive family history of sudden death at an early age — a similar ECG in close relatives. Circulation, 117, 1890–1893. [3]:
J Electrocardiology 45 (2012):433-442. Further history later: This patient personally has no further high risk features (syncope / presyncope), but her mother had sudden cardiacarrest in sleep. Bayes de Luna, A et al. Current electrocardiographic criteria for diagnosis of Brugada pattern: a consensus report.
Europace 2012 Shinde R, Shinde S, Makhale C, Grant P, Sathe S, Durairaj M, Lokhandwala Y, Di Diego J, Antzelevitch C. The final letter in the SLOWED mnemonic is " D " for "Dead" ( resulting from VT/VF or asystolic cardiacarrest ). Can J Cardiol 2022 Kukla P, Jastrezebski M, Praefort W.
The patient was unconscious BEFORE the cardiacarrest, at the same time that she had strong pulses. Therefore, cardiacarrest is NOT the etiology of the coma. More cases here to highlight: [link] Middle Aged Woman with Asystolic CardiacArrest, Resuscitated: Cath Lab? OMI is a clinical diagnosis.
